Medical Information: Athletes MUST submit all medical forms prior to try-outs in order to participate in athletics. Athletes that do not have the proper medical forms on file with the nurse will not be allowed to try-out.

Athletes need to have the following medical forms on file:

1. Parental/Medical Consent Form
2. State of Connecticut Department of Education Health Assessment Record or Connecticut Pre-Participation Form

• Note:  On the State of Connecticut Department of Education Health Assessment Record the section "This Student may: participate fully in athletic activities and competitive sports" must be checked off by the doctor.
